In collaboration with the primary care physician, the Geriatric Nurse Practitioner will provide primary health care to outpatients in the Hebrew SeniorLife primary care clinic at NBOC. Outpatient primary care will focus on disease prevention, wellness, and management of chronic and episodic illness.
